A case of SIADH with Pulmonary Tuberculosis and Tuberculous Meningitis

Abstract
It was well known that the syndrome of inapproprate secretion of antdiuretic hormone (SIADH) has been associated with many pulmonary diseases, cancers, diseases of the central nervous system, and drugs. But there were a few reports of the SIADH associated with pulmonary tuberculosis and tuberculous meningitis in spite of its high prevalence rate in Korea. We experienced a case of the SIADH associated with pulmonary tuberculoss with tuberculous meningitis. Before the start of therapy, his serum sodium concentration was 127 mEq/L, serum osmolality 265 mOsm/Kg., urinary sodium concentration 310 mEq/day, and the urine osmolality 590mOsm/Kg. The creatinine clearance and intravenous insulin tolerance test for adrenal reserve function was normal. No other causes of this SlADH were apparent. Neverthless, the most probable underlying cause of SIADH in this case was pulmonary tuberculosis combined with tuberculous meningitis. This was fully recovered after combined antituberculous therapy.
