Survey of the BCG Vaccination Practices

Abstract
A survey on the pediatrician's attitude toward BCG vaccination was carried out in 1984. The questionnaires were sent to all members of the Korean Pediatric Association (KPA). Out of 1,365 members, 606 pediatricians (44 % ) answered to the questionaire. 410 doctors (67.7% ) vaccinated r egularly;44 (7.3 % ) occasionally; and 152 (25 .1 %) were not vaccinating. Most of the respondents (96.5%) recognized the necessity of BCG vaccination in Korea. 110 (18.3 %) believed the high protectiveness; 355(59. 2% ) cosidered protective to some extent; and 24 (18.5 % ) not protective at all; and remaining 111 (1 8. 5%) answered “unknown" Two hundred and ten(36.4 % ) were performing Mantoux test regularly prior to vaccination; and 351 (60. 8% ) if necessary ; 16(2. 8 %) gave direct vaccination. Most prevalent sites of vaccination were upper arm and shoulder. The usual dose was 0.1ml of 1 mg/ ml of vaccine. One hundred and forty four (24.4 % ) reported frequent experiences of untoward reactions; 402(66.9 %) occasionally; and 55 (9. 0%) none. Out of 1, 377 reported untoward reactions, most frequent reactions were swelling of regional lymph nodes (36. 9%); followed by suppuration of the regional lymph nodes (29.9 %); ulceration of the vaccinated sites (29 . 3% ); and 34(2.5 % ) claimed pulmonary tuberculosis; 20 ( 1. 4% ) stated generalized BCGitits. However, it seems that the occurrence of pulmonary tuberculosis and generalized BCGitis might be misclassified or exaggerated. “Chemotherapy and dressing after rupture of the suppurated regional lymph nodes" was the most prevalent method of the treatment, followed by “ chemotherapy and dressing after observation". 60 % of the doctors medicated isoniazid alone; around 33% isoniazid and rifampicin to the children with untoward reaction, and the duration of the drug treatment being usually 4 to 6 months. In conclusion, the most pediatricians believed in the protective effect of BCG. However, many were reluctant to the BCG vaccination mainly due to untoward reactions, their occurrences seem to be over-estimated and exaggerated.
