A Study on Hospitalized Patients with Pulmonary Tuberculosis

Abstract
During the last 12 months from October 1988 to October 1989, We made study on 392 patients who were Hospitalized to our chest clinic with pulmonary tuberculosis on the basis of clinical history, sputum finding, and chest roentgenograms. The results were as follows: 1) The ratio of male to. female was 2. 7 to l. The highest incidence was seen in the 5th decade in male and 3rd decade in female. 2) By educational background, 120 (41 .9%) male patients finished a course of an elementary school, which was the highest incidence, 31 (29 .2%) female patients were not received education, which was the highest . 3) Occupational distribution were revealed as follows: Agriculture was 28%, business 18.4% , and jobless 5. 9%. The highest incidence was agriculture. 4) Living standard of them were reviewed as follows: 230 (58.7%) patients were lower level, only 12 (3.0%) patients were high level. 5) In order to seek first help, 247 (63%) patients visited health centers, 83 (21 . 2%) patients hospitals. 52(13.5%) patients private clinics, 9 (2.3%) ptients drug stores. 6) Causes of the failure and untoward reacti ons for previous chemotherapy were as follows: 145 (37%) patients discontinued the drug by reason of improvement of health condition, 85 (21.7%) patients irregular life. 119 (30 .3%) patients experienced gastrointestinal troubles, 48 (1 2. 2%) patients skin reactions, 45 (11.5%) patients arthralgia. 7) Total positive rate (smear positive and culture positive) was 55 .4% , culture positive rate was 12. 2% , smear positive rate was 3. 1% and total negative rate (smear nega tive and culture negative) was 29. 3%. 8) The relationship between sputum examination and radiological finding was observed as follows: Tota l positive group was found in 76. 7% of far advanced tuberculosis with cavity, culture positive group was found in 12.3%, of far advanced tuberculosis with cavity. 9) Analysis of drug sensitivity pattern of those positive on culture showed: 93.9% of patients were resistant to pyrazinamide, which was the highest incindence, 4.5% resistant to tuberactinomycin. the lowest incidence. 10) The total number of negative conversion by the duration of treatment were 226 (85.3%) of positive patients within 4 month. 158 (59.6%) patients within I month, 55 (20 , 8%) patints within 2month. 11) The rate of multiple drug resistance showed: Of 265 positive patients on culture. 97 (36 .6%) patients were resistant to 4 drugs. which was the highest. 24 (9.1 %) patients were resistant to 6 or more drugs. which was the lowest 12) The rate of negative conversion by multiple drug resistance were 100% for 2 drug-resistant group. 98.3% for 3 drug-resistant group. 88.7% for 4 drug-resistant group. 78. 4% for 5 drug resistant g roup. 41.7% for 6 or more drug-resistant group. which was the lowest. The more resistance of drugs was. the lower rate of negative conversion was.
